Time Expectation:

Purpose: This assignment is designed to help you think clinically rather than memorize. By creating your own case, you will strengthen:

●      Clinical reasoning

●      Recognition of fluid and electrolyte imbalances

●      Test-taking skills

Instructions:

You will create TWO original patient cases:

  1. ONE electrolyte imbalance

  2. ONE fluid imbalance

Each case must follow the structure below.

PART 1: Create Your Case

Choose ONE electrolyte imbalance:

●      Hyponatremia or Hypernatremia

●      Hypokalemia or Hyperkalemia

●      Hypocalcemia or Hypercalcemia

AND choose ONE fluid imbalance:

●      Fluid volume deficit (dehydration)

●      Fluid volume excess (overload)

Case Template (Complete for EACH case)

Patient Scenario:

Write 3–5 sentences describing your patient.

Include:

●      Age and basic background

●      Reason for hospital visit

●      Relevant history (if applicable)

●      Be creative and make it fun!

Guiding questions:

●      What caused this imbalance?

●      What is happening in the body?

Labs / Key Data:

List at least 2–3 findings:

●      Electrolyte value(s)

●      Vital signs and/or physical findings

Guiding questions:

●      What lab value confirms the imbalance?

●      What clinical signs would you expect?

Signs and Symptoms:

List 3–4 symptoms the patient is experiencing

Guiding question:

●      What would you observe in this patient?

Priority Concern:

Identify the biggest risk for this patient

Guiding question:

●      What could harm this patient first?

Priority Intervention:

Identify the first or most important action

Guiding question:

●      What should the nurse do immediately?

PART 2: Reflection

Answer the following:

●      What concept do you understand better after completing this assignment?

●      What is still confusing?

●      What mistake are you less likely to make now?
